They act as instructions for families, surrogates, and healthcare professionals. There are forms for advanced directives as well as end of life documents. It is important to have all affairs in order before you become too ill to do so – otherwise you have no guarantee that your wishes will be honored. These forms let you outline who is in charge of making medical decisions in case you are incapacitated, what kind of life saving measures you want taken (if any), and what happens in any extreme circumstances. Be sure to choose someone who is going to advocate for you and have your best interests in mind and that you trust to follow your directives.
